The New Jersey Healthy Communities Network’s Community Grants Program brings together local, regional, and statewide funders, leaders and partners to support communities in implementing healthy eating and active living strategies to advance environment, policy and system changes.

Since 2011, the NJHCN Community Grants Program has provided $3.7 million in grants. The NJHCN 2020 Community Grants Program funding collaborative consists of: Atlantic Health System, New Jersey Department of Health, New Jersey Division of Disability Services New Jersey Health Initiatives, Partners for Health Foundation, The Russell Berrie Foundation and Salem Health & Wellness Foundation. NJ SNAP Ed provides additional infrastructure support.

Evaluation for the Community Grants Program is conducted by Center for Research and Evaluation on Education and Human Services (CREEHS) at Montclair State University.  Key findings from the 2018-2019 grant cycle is detailed in the Evaluation Key Findings – April 2020
